* Do not list your school (@uth.tmc.edu) address. Enter a home/personal email address that will not expire within a year. Make sure the email address is also in myUTH. Your home/personal email address is used to communicate post-graduation information such as diploma delivery and commencement information.
To update academic information, you must submit the appropriate form to the Office of Academic Affairs and Student Services.

Each member listed must be officially appointed with the Office of Academic Affairs and Student Services.
