What is the purpose of this tool?

The purpose of the curriculum log is to document the number of lessons and activities completed, the number of students who participated in each of the lessons, and changes made to any of the activities for each lesson during the implementation of the curriculum. The information gathered will be used to help you identify challenges for implementation and understand how much fidelity was maintained during the curriculum’s implementation.

Who should complete these templates?

The program facilitator/teacher for the lesson should complete the curriculum log at the end of each class period in which the curriculum is taught.

How long will it take to complete curriculum logs?

This may vary depending on the number of notes that need to be added for activity and lesson changes. However, generally, it should only take a few minutes to complete a curriculum log.

What do I need to complete curriculum logs?

You will need the attendance log for each class period and any notes for activity or lesson changes.
