Who We Are

The Coalition for Connected Care (CCC) is a systems coalition dedicated to enhancing care coordination for non-medical drivers of health in the Greater Houston region. The Coalition has over 1,500 members representing 385 health systems, payers, social service organizations, and academic institutions. The Coalition brings these groups together under a shared plan, using a collective impact approach.

Members align their efforts to strengthen our region's infrastructure and enhance its capacity to achieve improved health outcomes.

The Center for Healthy Communities serves as the backbone organization, anchoring this collective impact effort. Learn more about the Coalition's approach and its Framework for Driving Better Health Together

We host monthly themed networking sessions, as well as regular learning opportunities and workshops, to promote a shared learning environment that incorporates perspectives from local, state, and national experts. Check out the events calendar for upcoming seminars, sessions, and other events.

Strengthening the Community Health Workers Ecosystem

In partnership with the City of Houston Health Department, we’ve gathered stakeholders across fields to advance CHWs’ priorities, inform the implementation of legislation, increase training opportunities, and dissipate findings.

Implementing a Closed-Loop Referral Network

The Closed Loop Referral Network creates a bi-directional flow between healthcare and social service data systems to improve care coordination.

In collaboration with the Center for Healthy Communities, the following resources have been developed or collected to support the building of a comprehensive Coalition for Connected Care Network.
