What is the purpose of the Guidance Sheet and Maintenance Plan?

After an approved evidence-based sexual health education program has been implemented and evaluated, the results of the evaluation should be used to inform (1) whether the district should continue to implement the program and (2) areas where implementation could be improved. It is also important to maintain district and SHAC support for sexual health education. This support is especially crucial if it is decided that the selected program is not a good fit and another evidence-based program is to be selected.

The Guidance Sheet helps identify targets for remediation as well as potential next steps for the district.

The Maintenance Plan helps you identify strategies to sustain positive support, with the ultimate goal of institutionalizing evidence-based sexual health education in the district.

Note: The Guidance Sheet and Maintenance Plan should be used in conjunction.

Who should complete these templates?

Any persons in charge of overseeing the implementation of the selected evidence-based sexual health education program and any persons in charge of reporting back to the school board and SHAC should be involved in completing the Guidance Sheet and Maintenance Plan. There should be back-and-forth communication between the district and school levels to ensure mutual understanding of the plan and a smooth collaboration throughout implementation.

Note: The Guidance Sheet and Maintenance Plan should be completed for EACH site/school that implemented the approved evidence-based sexual health education program.
