CHPPR and Epilepsy Foundation Texas are testing a web-based epilepsy management support tool as a Community Health Worker delivered intervention in clinics. Health workers can use the tool to support patients' epilepsy self-management while also providing linkage to additional services.

Lifestyle Change Programs are a key component of the National Diabetes Prevention Program, which CHPPR is helping Texas clinics to implement. These programs rely on Lifestyle Coaches to help participants make and maintain the lifestyle changes that can help prevent type 2 diabetes.

The Epilepsy Foundation Texas is a leader in the treatment and support of people living with epilepsy, offering a variety of services including medical clinics, support groups, summer camps, and community education throughout the state. They also partner with CHPPR to implement MINDSET, a decision-support program for epilepsy management, in clinics.

The CHPPR e-Learning program Project STOP is a smoking cessation program for health care providers. As one of the aims of this project, the team designed a smoking cessation smartphone application, DECISION-t (D-t), will help providers make quicker, better decisions about smoking cessation treatment approaches for their patients.
