Healthy Fit.

Project Overview

Healthy Fit connects residents in predominantly low income Hispanic communities to health resources. Community health workers administer a health screening and then use motivational interviewing to help participants follow through on recommendations. The program provides vouchers for breast, cervical, and colorectal cancer screenings, along with flu, pneumonia, and HPV immunizations. Qualifying participants receive heart health fotonovelas and referrals to physical activity opportunities and a weight loss support group.
