The Diabetes Prevention Program

A Lifestyle Change Weight-Loss Program to Help Prevent or Delay Type 2 Diabetes

Join OUR Certified Lifestyle Coaches to Work Together to Improve YOUR Health!

The Diabetes Prevention Program provides education, encouragement, and the tools necessary to help individuals reach their healthy lifestyle goals. This program is designed for non–diabetic, overweight individuals age 18 and older who have pre–diabetes and/or the metabolic syndrome, and are at risk for developing diabetes.

The goals of the Diabetes Prevention Program are to:

  • Reduce body weight by 5-7%
  • Increase Physical Activity by 150 minutes/week
  • Reduce HbA1c by 0.2%

Individuals who enroll in the Diabetes Prevention Program will work on:

  • Healthy Eating Habits
  • Grocery Shopping Tips/Meal Planning
  • Mindfulness & Stress Management
  • Safe Physical Activity
  • Goal Setting, Problem Solving, and/or Motivation
